Life Care Services, An LCS Company, has announced that 37 of its managed communities received a five-star rating from the Centers for Medicare and Medicaid Services (CMS). The five-star rating is the highest possible rating CMS awards to nursing home health centers based on their performance on health inspections, staffing levels, and quality measures. Only the top 10 percent of nursing homes nationwide achieve this distinction.

Using December 2023 data, The Forum is one of 37 Life Care Services Communities recognized with 5-star ratings.

The CMS star rating uses publicly available Medicare data to evaluate quality information across five care categories into a single-star rating. Categories include mortality, safety of care, readmission, patient experience, and timely and effective care.

Life Care Services is the nation’s third-largest operator of senior living communities, providing solutions to 140 communities and more than 40,000 seniors.
